Notice of Privacy Practices

Thrive Rheumatology and Arthritis Center is committed to protecting the privacy of your protected health information (PHI) in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and applicable state law. We are required by law to maintain the privacy of your PHI and to provide you with this notice of our legal duties and privacy practices.

We use and disclose your health information for treatment, payment, and health care operations — for example, to coordinate care with your primary physician, bill your insurance carrier, and evaluate the quality of care you receive. We may also disclose your information as required or permitted by law, including public health reporting, court orders, and to prevent a serious threat to health or safety.

• Request confidential communications (for example, contact you only by phone or only at a specific address).
• Ask us to limit certain uses and disclosures of your information.
• Receive an accounting of certain disclosures we have made.
• Receive a paper copy of this notice at any time.
• File a complaint with our office or with the U.S. Department of Health and Human Services if you believe your privacy rights have been violated. We will not retaliate against you for filing a complaint.

We use administrative, physical, and technical safeguards to protect your health information — including secure electronic records, restricted access, encrypted communication where available, and staff training on privacy and security.

We may update this notice from time to time. The updated notice will apply to all health information we maintain. The current notice will always be available in our office and on this page.
